All applicants who completed the form earlier should used the same phone number to register and they will get access to the form and fill again with same information. There is no need to print and sign if you already did that, you can submit the old signed form.
Stage 1: CHECKING CLEARANCE
To check clearance, visit https://hr.moh.gov.gh/check-clearance
1. Enter Your N&MC Index Number: 00000001
2. Enter Your Full Name in the second box: Darko Samuel Okyere
3. Click on [ SUBMIT] (Your index number, Name, Institution Attended and Grade will appear to indicate you are cleared)
Stage 2: CREATE ACCOUNT & LOGIN
After the system confirms you have been financially cleared, go to the signup/Apply Now page and create an account (https://hr.moh.gov.gh/register)
1. Enter Your N&MC Index Number: 30000002
2. Enter Your Full Name: Darko Samuel Okyere
3. Enter Your Email Address: slyzombe@gmail.com
4. Enter a preferred password ( and should be at least 8 characters): 0202020202GH
5. Confirm Your password (Enter the same password you typed earlier)
6. Click on [ REGISTER ]
Stage 3: THE DASHBOARD
Click on ‘Apply’ /’Log in’ from the left side of the page (From the Menu) to begin with the Whole Registration
